The place to file reports of sexually transmitted diseases to both the local health officer and the State for laboratories, physicians and hospitals in Boone, Hamilton, Hancock, Hendricks, Johnson, Marion, Morgan and Shelby Counties, is the Bell Flower Clinic (must be so marked) 1101 West 10th Street, Indianapolis, IN 46202.

Report cases by mail or telephone using official report forms furnished by the State Board of Health. We would prefer to be notified by FAX (221-8301) because this is consistently the most rapid method and facilitates documentation.

If the 72 hour deadline falls on a holiday or weekend, reports should be sent before the close of the business prior to the weekend or holiday period. If unable to do so, please file STD reports by FAX on the next business day.
